Skilled Nursing Care

Our skilled nursing services such as medication management are provided by Certified Medication Technicians (CMT) or LPNNs that are overseen by an RN.

Companion Care

Our companion care focuses on providing emotional support and companionship to seniors who are generally healthy and wish to maintain their independence at home.

Personal Care

Personal Care Services are provided by nursing aides, personal care providers or home health aides. We support client with Activities of Daily Living (ADL)

Easy Steps To Get Our Services

Initial Consulation

A HSS representative will either meet in person or engage in a phone discussion with the client and their family to evaluate their needs and preferences, ensuring a comprehensive understanding of the required services.

Personalized Care Plan

Using information gathered during the initial consultation, a personalized care plan is created, outlining the specific services, schedules, and goals tailored to meet the client's unique needs.

Caregiver Matching

We meticulously pair a skilled caregiver with the client, considering their expertise, experience, personality, and compatibility. This ensures the establishment of a positive and comfortable caregiving relationship.

Care Services Begin

Upon the completion of caregiver matching, HSS commences the services, delivering the client the dedicated care and support detailed in the personalized care plan.
